RARE TOGETHER

YWHAG Research Foundation has joined the Rare Patient Network, powered by the Ciitizen platform, a digital Natural History Study that organizes, summarizes and stores patient medical record information obtained from multiple hospitals and providers.

Test results and vital information which may expedite research and treatment protocols are often trapped in medical records that are stored across several, disparate locations IRL or digitally.  This can be time consuming to retrieve in moments of emergency, especially with patients that have acquired voluminous amounts of data and test results surrounding their condition.

With Ciitizen-- as a patient or parent/guardian, one can unlock these records and enable research in a private and secure way-- instantaneously. Ciitizen is designed to be leveraged as a transparent and central catch-all, and to facilitate cross-provider access in critical, time-effective manners.

Ciitizen is an easy-to-use digital platform to store and retrieve medical records in a single location and is accessible anywhere, at any time.
